Medical Enquiries – Online Consultation Form

For all medical enquiries, please complete our secure online consultation form.

The form takes only a few minutes to complete and is available for patients aged 6 months and over.

How Our Clinical Triage System Works

Our practice operates a 100% clinician-led triage system. This means that every medical request submitted through our online form is reviewed directly by a qualified clinician to ensure patients receive the most appropriate and timely care.

Once your form is submitted:

  • Your request comes straight to the practice.
  • A clinician carefully reviews (triages) the information provided.
  • An appointment or outcome is arranged based on clinical need.
  • You will be contacted and informed of the next steps.

This approach allows our clinicians to gather detailed information in advance, helping them make safe and informed decisions about your care. It also ensures appointments are prioritised appropriately.

Types of Appointments

Following triage, you may be offered:

  • A face-to-face appointment, if clinically required or requested
  • A telephone consultation
  • Advice via text message or secure message
  • Support with administrative requests (e.g. test results, fit/sick notes)

Face-to-face appointments are always available where clinically necessary or if you would prefer one.

If You Cannot Complete the Online Form

We understand that not everyone has access to the internet or feels confident completing online forms.

If you:

  • Do not have access to a computer, smartphone, or tablet
  • Are unable to use online services
  • Require assistance completing the form

You can:

  • Visit the reception desk at the surgery, or
  • Telephone us on 01262 670686, where a trained member of our reception team will complete the form on your behalf.

All telephone and in-person requests are entered into the same system and will be assessed by a clinician in exactly the same way as online submissions.

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

If you need help with your appointment

Please tell us on the online form:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.

To request a home visit, it’s helpful if you phone the practice as soon after 8am as possible, and before 10am.
